Wellness Coach (Remote)
Job Description
This job has primary ownership and oversight over a specified panel of members that range in health status/severity and clinical needs. The incumbent assesses health management needs and utilizes data/analytics to identify the right clinical/lifestyle intervention for each member. The incumbent conducts outreach to members enrolled in disease management, developing care plans and encouraging behavior changes.
Responsibilities
- Deliver lifestyle coaching in tobacco cessation, weight management, sleep hygiene, and nutrition
- Maintain oversight over specified panel of members
- Create care plans to address members’ identified needs and remove barriers
- Demonstrate clinical outcomes and cost savings
- Ensure all activities are documented and compliant with regulatory requirements
Qualifications
- Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology
- 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach
